In case other people may be interested in the answer to those questions:
I booked with Pilgrim Tours, cost about 1000EUR, including 2 nights' hotel in Moscow, all domestic flight and transfer, all lodging/food/guide in the mountains. Seems to be the best deal I could find.
We meet the guide in Mineralnye Vody (Caucasus) airport.
The best season to climb there is mid July - mid Aug. a couple weeks before or after that is still good. But weather is very changeable, even in the prime season.
