I did RAGBRAI a few years ago and there was a fellow riding one - a rather tall one, with pedals about 3' off the ground. I never saw him go downhill, but he must have, as there are lots of them in the 550 miles or so across Iowa. Apparently he'd done the ride several times in the past. Am going to do it again this summer, and will be curious to see if he's also there.
A running friend of mine from Alabama is also into mountain/trail unicycling.
He rode down Mt. Elbert (Colorado's highest peak) a couple summers ago!

I watch this on my 52 inch Vizeo Smart TV and get chills every time. Why did I not find this sport when I was about 20 years younger?
